Your First Visit
Please arrive 10 minutes early so you have enough time to fill out a New Patient form. You can also download and print the form to fill out before your arrival.
Initial Examination
Your dentist will review your medical and dental history and complete a comprehensive exam to check your teeth, bite and jaw. We will also conduct an oral cancer screening and may recommend other diagnostic procedures, such as X-rays.
Treatment Plan
Based on your history and examination, your dentist will develop a long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.

How do I find Live Well Dental in The Well building?
From the North-West corner of Front and Spadina, take the escalator up to the 2nd floor and you’ll find us there on the left side.

Is the office wheelchair accessible?
Yes. Enter The Well from the North-West corner of Front and Spadina. Pass the escalators until you see an elevator sign on the right side. Take the elevator to Floor 2, and when exiting the elevator turn left and cross the bridge. Enter the Live Well main doors.

To cancel or reschedule an appointment, please call us. Please let us know about any changes in plans as soon as you can, so we can offer the space in our schedule to another patient.
